Smiles for Ohio
CHANGING LIVES IN OHIO, ONE PATIENT AT A TIME
The goal of the “Smiles for Ohio: Give Back to Ohio” program is to change lives in Ohio, one patient at a time. We are currently in the process of fundraising for this program to make it happen.
The Asian Pacific Dental Alumni Society at the Ohio State University and our smile makeover giveaway sponsors are pleased to present you with an opportunity to win the smile makeover. Best dental specialists in Ohio will take care of your treatment. If you win our smile makeover, your dental treatment will be paid for.
Giveaway is open to any Ohio resident, 18 years or older, who is below the 250% poverty level with no dental insurance. Tell us what makes your case special and how a new smile would change your life.
How does it work?
Submit the following:
APDAS Smile Makeover Application
Your Essay (500 words or less)
1 to 5 photographs of you or your nominee

We will start accepting application via email to Isi Ikharebha Green with Physicians CareConnection. Isi’s email is isi@pcchealth.org.
